LIVE WELL Workshops
For Individuals with Chronic Conditions or Their Caregivers
Limited number of participants. Please call us to ask questions about the program: (860) 567-6000 or (860)-364-4424. The program covers a new topic each week and provides opportunities for interaction and group problem solving. Topics include: Managing Difficult Emotions, Better Breathing, Communication, Fitness and Exercise, Nutrition, and Working with your Health Care Professional.

2017 Fall Risk Assessment Clinics
(Open to the Public at no charge — funded by a grant from the Connecticut Collaboration for Fall Prevention )
During this program a therapist will describe various risk factors that can lead to a fall and will explain and demonstrate some safety measuers and tools that individuals can use to protect themselves from accident or injury. Following the presentation there will be an opportunity for participants to receive personal reviews of their medications and blood pressure checks by a nurse, as well as the opportunity for individual fall risk assessments by a therapist.

No reservations are necessary.
Cost without insurance: $35. Pneumonia shots: $80. Bring insurance cards if you have Medicare Part B, Aetna, Anthem, or ConnectiCare. Please wear short sleeves.
